Iron (Fe) is a transition element that plays an important role in the oxidant/anti-oxidant balance and a co-factor of various enzymes. According to epidemiological data, environmental and occupational agents play a critical role in the development of interstitial lung diseases (ILDs), such as idiopathic interstitial pneumonia (IIPs), non-specific interstitial pneumonia (NSIP) and sarcoidosis. A study investigated the exhaled breath condensate (EBC) of 33 healthy subjects, 22 patients with sarcoidosis, 15 patients with NSIP and 19 with IIPs to determine the role of Fe metal in the pathogenesis of ILDs. Fe traces were measured by inductively coupled plasma-mass spectrometry (ICP-MS). The report indicates Fe levels were higher in the control subjects than in the IPF and NSIP patients. There was a positive correlation between EBC Fe levels and diffusing lung capacity for carbon monoxide (DLCO) in the patients with sarcoidosis, and between EBC Fe levels and total lung capacity (TLC) in the IPF patients. COPD patients showed lower levels of Fe, and patients with asthma had lower levels of Fe than healthy non-smokers. Chronic inflammation and oxidative stress are important mechanisms in the pathogenesis of ILDs. ILD patients have lower Fe levels, compared to controls. This reduction in Fe was possibly due to pulmonary depletion as a result of chronic oxidative stress and inflammation. Assessing the elemental composition of EBC in ILD patients seems to provide useful information, although further studies are required to confirm the robustness of this approach, and to better understand the underlying pathophysiological processes of ILDs ( J Breath Res.
