Isobutyrate arises from valine catabolism. Nuclear magnetic resonance (NMR)-based metabolomics has the ability to separate exhaled breath condensate (EBC) profiles of patients affected by pulmonary diseases from those of healthy subjects. EBC has been used to study the influence of smoking on changes in smoking-related airways diseases, but data on metabolic variation in different diseases sharing a common risk factor: smoking, hasn't been established. In a study, 50 duplicated EBC samples from smokers (without COPD, with COPD, or with pulmonary Langerhans cell histiocytosis (PLCH)) were analyzed by means of NMR spectroscopy followed by principal component analysis (PCA) and projection to latent structures discriminant analysis (PLSDA). The result shows clusterization of EBC spectra was disease-specific; COPD and PLCH samples present a profile different from that of HS (smokes without COPD). Smokers with COPD correlate with low isobutyrate, whereas smokers with PLCH correlate with high isobutyrate. Isobutyrate level is different in COPD and PLCH, the relationship is not clear, but the NMR approach could favor a noninvasive monitoring of both pathologies (J Proteome Res. (2013)).
