Asthma has been recognized as a heterogeneous disease, having genetic and environmental components and is a complex multifactorial disorder. The metabolomic approach using exhaled breath condensate (EBC) from children patients with severe asthma can be used to discriminate different asthma phenotypes. 42 asthmatic children in 8-17 years old were recruited (31 with nonsevere asthma and 11 with severe asthma), and additionally 15 healthy children were collected as control set. Exhaled breath condensate (EBC) of these children was collected using exhaled nitric oxide measurement, spirometry and the corresponding condensation of EBC was analyzed by mass spectrometry. A Bidirectional-Orthogonal Projections to Latent Structures-Discriminant Analysis (O2PLS-DA) model was established for classification between severe asthma children and healthy subjects, and the model which includes the concentration of retinoic acid, deoxyadenosine and vitamin D as variables achieved R2 value of 0.93 and Q2 value of 0.75. Another best O2PLS-DA model including the concentration of retinoic acid, deoxyadenosine and vitamin D as variables for distinguishing severe asthma and nonsevere asthma achieved R2 value of 0.84 and Q2 value of 0.47. Furthermore, a robust robust model was also built for clustering 3 groups of children. It showed that each group can be characterized by EBC profile (Allergy. (2013)). Retinoic acid, adenosine and vitamin D were all important chemicals for the classification between groups. For discrimination severe asthma subjects with healthy controls, retinoic acid is a key compound. A recent literature also suggested that retinoic acid and its metabolites play an important role in both inflammation (J Immunol. (2008)) and airway remodeling of asthma (Am J Respir Cell Mol Biol. (2008)). EBC profiling applying metabolomic approach is a useful method for characterization of different specific metabolic fingerprint in asthmatic children.
