According to epidemiological data, environmental and occupational agents play a critical role in the development of interstitial lung diseases (ILDs), such as idiopathic interstitial pneumonia (IIPs), non-specific interstitial pneumonia (NSIP) and sarcoidosis. A study investigated the exhaled breath condensate (EBC) of 33 healthy subjects, 22 patients with sarcoidosis, 15 patients with NSIP and 19 with IIPs to determine the role of cobalt (Co) in the pathogenesis of ILDs. Co traces were measured by inductively coupled plasma-mass spectrometry (ICP-MS). The report indicates Co levels were below the limit of detection (LOD) in all of the ILD patients. But the trend seems to suggest the EBC levels of pneumotoxic Co metal were higher in controls compared to IPF and sarcoidosis patients. This study concludes Co distinguished ILDs patients from healthy nonsmokers with relatively high degress of sensitivity (96.4%) and specificity (90.9%). Assessing the elemental composition of EBC in ILD patients seems to provide useful information, although further studies are required to confirm the robustness of this approach, and to better understand the underlying pathophysiological processes of ILDs ( J Breath Res. Non-specific interstitial pneumonia (NSIP)

No significant difference of Co metal level in between NSIP patients and healthy control (J Breath Res. (2009)).
